Following a 14 week (December 2018 to March 2019) public consultation by North Staffordshire and Stoke on Trent Clinical Commissioning Groups, a decision was made to introduce Integrated Care Hubs across North Staffordshire. 
Four hubs will be developed in 
1.    Leek Hospital
2.    Bradwell Hospital
3.    Haywood Hospital
4.    A location within the Longton area

The development of Integrated Care Hubs will see people with health and social care needs be treated seamlessly.

Health and social care teams will work alongside each other and be based within hubs to ensure individuals received joined-up care. 

Local residents across North Staffordshire will be involved in designing the Integrated Care Hubs through robust and comprehensive engagement. 

Each hub will be reflective of the needs of the population which they serve.
