We are working closely with fellow NHS organisations, local authorities, voluntary and community organisations and people with lived experience to transform the way adult community mental health care and support is delivered locally.

The transformation forms part of a national programme set out in the NHS Long Term Plan to enable adults with severe mental illness to access services in a new, more joined up and effective way, regardless of their diagnosis or level of complexity.

This is about offering flexible and personalised care and support that responds to a person’s mental health needs and preferences close to home, while also increasing support for the wider factors that can impact someone’s wellbeing, such as employment, housing and physical health.

To do this, partners are working together more closely than ever before, based within networks of local GP practices (known as Primary Care Networks).

NHS England has developed the following short animation that provides a helpful explanation of how this transformation will happen.

MPFT is a leading partner in the transformation in Southern Staffordshire (as part of the wider Staffordshire & Stoke-on-Trent programme) and Shropshire and Telford & Wrekin – click below to find out more about how you can get involved in these areas.
